Please make sure you have this form submitted before your appointment, including your insurance information, if you want to use your Extended Health.

Yes, we provide direct billing to the majority of insurance companies.
Yes, our practitioners accept and direct bill ICBC patients.
We are required 24 hours notice for any cancellation. Patients who provide less than 24 hours notice or missed their appointment, will be charged a cancellation fee equal to the rate of missed treatment.
